Asthma in pediatric (<12 ปี)


Epidemiology, initial evaluation, and diagnosis

INTRODUCTION

  • Asthma เป็น chronic disease ที่พบบ่อยมากในเด็กทั่วโลก (prevalence ประมาณ 1–18% แล้วแต่ประเทศ)
  • การวินิจฉัยต้องอาศัย history + physical exam + diagnostic studies และต้องคิด differential diagnosis ของ wheeze/cough โดยเฉพาะ infants/young children ที่ทำ spirometry เพื่อพิสูจน์ reversible airflow obstruction ทำได้ยากและไม่ routine
  • เป้าหมายการวินิจฉัย:

1.       พิสูจน์ variable expiratory airflow limitation (ถ้าทำได้)

2.       มี reversibility

3.       exclude alternative diagnoses


EPIDEMIOLOGY (ภาพรวมที่ควรรู้)

  • Asthma prevalence แตกต่างกันมากระหว่างประเทศ (มักสูงในประเทศรายได้สูง)
  • ในสหรัฐฯมีแนวโน้ม “เพิ่มขึ้นในยุค 1980–1990” แล้วชะลอ/plateau ในช่วงหลัง; ปี 2020 prevalence เด็กประมาณ ~5.8% (ลดจากช่วงก่อนหน้าในหลายรายงาน)
  • Disparities ยังชัด: พบสูงกว่าในกลุ่ม socioeconomic ต่ำ/urban; ใน US พบสูงใน Puerto Rican และ non-Hispanic Black children
  • เพศก่อน puberty: boys > girls แต่ช่วง adolescence มักกลับทิศ
  • Exacerbation rates ในเด็ก US ลดลงช่วงหนึ่ง แต่ยังมีความผันผวนตามปี/ปัจจัยแวดล้อม

HISTORY (หัวใจของการวินิจฉัย)

โฟกัส 4 แกน: symptoms, pattern, triggers/atopy, risk factors

Symptoms ที่พบบ่อย

  • Cough และ wheeze (พบบ่อยสุด)
  • Breathlessness, chest tightness/pressure, chest pain
  • Poor school performance/fatigue อาจสะท้อน nocturnal symptoms

Cough pattern ที่ชวนคิด asthma

  • Nocturnal cough
  • Seasonal recurrent cough
  • Triggered by cold air/exercise/laughing/allergen/crying
  • Cough >3 weeks (โดยเฉพาะหลัง URI)

เด็กบางราย “cough เป็นอาการเดียว” (cough-variant asthma)

Wheeze pattern ที่ช่วยแยกโรค

  • Asthma wheeze มัก polyphonic
  • Severe obstruction อาจได้ทั้ง insp+exp wheeze
  • Monophonic wheeze ชวนคิด central airway obstruction (เช่น tracheomalacia)
  • Inspiratory monophonic/stridor ชวนคิด upper airway obstruction (เช่น vocal cord dysfunction/ILO)
  • Silent chest ระหว่าง exacerbation = medical emergency

Pattern ของอาการ (typical patterns)

  • Intermittent exacerbations มีช่วงปกติคั่น
  • Chronic symptoms มีช่วงแย่เป็นพัก ๆ
  • Morning “dipping” (เช้าแย่ กลางวันดีขึ้น)

Triggers/precipitants ที่สำคัญ

  • Viral URIs = trigger สำคัญที่สุดทุกวัย (รวม infants)
  • Exercise-induced bronchoconstriction (EIB): พบบ่อยมากในเด็ก asthmatic และอาจเป็นอาการเดียว
    • มักเริ่มระหว่างออกกำลังต่อเนื่อง/หลังหยุดออกกำลัง 5–10 นาที และค่อยกลับ baseline ภายใน 30–45 นาที
  • Weather (cold air, hot humid, thunderstorm ฯลฯ)
  • Secondhand tobacco smoke = external risk factor สำคัญมาก
  • Vaping/secondhand e-cigarette aerosol: มีข้อมูลสนับสนุนว่าเกี่ยวข้องกับ pulmonary function/inflammation และ exacerbation odds
  • Allergens (dust mite, cockroach/rodent, pets, pollens, molds)
  • Indoor irritants: NO₂ จาก gas stove, smoke/particulates, sprays, perfumes, cleaning products ฯลฯ
  • Stress: เป็น trigger ได้ แต่ไม่ควรโทษ stress จนกว่าจะ exclude ปัจจัยอื่น + ต้องคุม asthma ให้ดีพอ

Additional history ที่ “ต้องถามเพิ่ม”

  • Personal atopy (eczema, allergic rhinitis), food allergy
  • Family history of asthma/atopy (เพิ่ม pretest probability)
  • Environment ทุกที่ที่เด็กอยู่: home/school/daycare/ญาติ
  • Past medical history: neonatal course, FTT, recurrent infections, systemic symptoms → ชวนคิดโรคอื่น
  • Comorbidities: OSA, GERD, chronic rhinosinusitis
  • Medication history: adherence, technique, dosing interval, response to albuterol
  • Health care utilization: ED/hospital/PICU, oral steroid bursts → บ่งชี้ poor control และ future risk
  • School attendance/performance, physical activity limitation
  • Psychosocial factors (access to care, family stress, financial burden)

PHYSICAL EXAMINATION

  • ระหว่าง exacerbation: tachypnea, hypoxia, wheeze, retractions, prolonged expiratory phase (รายละเอียดแยกจาก acute management)
  • ระหว่างไม่กำเริบ: มักปกติ
    ถ้ามี abnormal signs ตอน baseline → ชวนคิด severe disease/suboptimal control/comorbidity เช่น
    • wheeze/decreased air entry, prolonged expiratory phase
    • allergic rhinitis/sinusitis signs, eczema
    • nasal polyps (ในเด็กควรนึกถึง CF/primary ciliary dyskinesia)
    • increased AP diameter (air trapping)
  • Obesity: สัมพันธ์กับ asthma incidence/severity ได้ แต่ causality ยังไม่ชัด (reverse causation เป็นไปได้)

DIAGNOSIS (แกนหลัก 3 ข้อ)

วินิจฉัย asthma เมื่อมี compatible history + (เมื่อทำได้) มีหลักฐานว่า:

1.       Variable expiratory airflow limitation

2.       Reversible obstruction

3.       Exclusion of alternative diagnoses

Spirometry ปกติ หรือไม่มี reversibility ระหว่าง exacerbation ไม่ตัดทิ้ง asthma
ถ้าสงสัยมาก → ทำ trial of asthma medication และดู response


SPIROMETRY (preferred test; ทำในเด็ก ≥5 ปีเมื่อสงสัย)

  • วัด FVC, FEV1, FEV1/FVC
  • นิยาม obstruction ที่ใช้บ่อย: FEV1 <80% predicted และ FEV1/FVC <0.85 (อ้างอิงตาม age/height/sex/race)
  • Bronchodilator response (BDR): ทำ pre/post bronchodilator
    • เกณฑ์ classic: FEV1 ↑ ≥12% (จาก adult data)
    • ในเด็กอาจใช้ cut-off ≥8% ได้เหมาะกว่าในบางงาน
  • FEV1/FVC มัก sensitive ต่อ impairment; FEV1 มีความสัมพันธ์กับ risk ของ future exacerbation
  • FEF25–75 อาจช่วยในเด็กที่ FEV1 ปกติแต่สงสัย small-airway involvement (ยังต้องหลักฐานเพิ่ม)
  • Peak flow: effort-dependent/variability สูง → ไม่ใช้เดี่ยว ๆ เพื่อ “diagnose” (ใช้ติดตามได้ แต่ serial spirometry ดีกว่า)

เด็ก <5 ปี

  • Spirometry มักทำไม่ได้ → ใช้ trial of therapy + clinical response เป็นหลัก
  • Impulse oscillometry (IOS) เป็นทางเลือก (ต้อง cooperation แบบ passive) แต่ไม่ widely available

ANCILLARY STUDIES (ใช้ “เลือกเคส” ไม่ใช่ routine)

  • Allergy testing (skin/in vitro): มีประโยชน์เมื่อประวัติสิ่งแวดล้อมชี้ชัด เพื่อวาง avoidance; food allergy testing ทำเมื่อมี story ที่สอดคล้องจริง
  • FeNO: ใช้เป็น adjunct เมื่อ diagnosis uncertain
  • Bronchoprovocation testing (methacholine/cold air/exercise): เมื่ออาการชวนคิด asthma แต่ spirometry ปกติ + ไม่ตอบสนองต่อ trial
    • ต้องทำใน specialized facility
    • หลีกเลี่ยงถ้า FEV1 <50% predicted
  • Chest radiograph: แนะนำเมื่อ ไม่ตอบสนองต่อ initial therapy หรือสงสัย alternative dx/complication
  • Sweat chloride test: low threshold เมื่อมีภาพเข้ากับ CF (malabsorption/FTT/recurrent pneumonia ฯลฯ)
  • Modified barium swallow: เมื่อสงสัย aspiration/swallowing dysfunction

DIFFERENTIAL DIAGNOSIS

  • Wheeze/cough ไม่ใช่ pathognomonic ของ asthma
  • ต้องคิดโรคอื่น โดยเฉพาะใน very young children และเมื่อ:
    • fail to respond ต่อ asthma therapy
    • history/physical มี red flags ของ alternative dx
      ตัวอย่างกลุ่มที่ต้องนึกถึง: upper airway obstruction/ILO, foreign body, congenital airway anomalies, vascular ring, CF, aspiration/GERD-related, chronic infection, PCD, tracheomalacia ฯลฯ

INDICATIONS FOR REFERRAL (asthma specialist)

ส่งต่อ pulmonologist/allergist เมื่อ:

  • Diagnosis uncertain
  • Difficult-to-control asthma / frequent exacerbations
  • Medication side effects problematic
  • Suspect alternative pulmonary disease / ต้องทำ advanced testing/bronchoscopy
  • ต้อง evaluate allergic triggers/food allergy หรือคุม allergic rhinitis/conjunctivitis ยาก

Assessment of severity/control, การเริ่ม controller, และการติดตาม/ปรับยา ในเด็ก <12 ปี

CONCEPT หลัก

  • การรักษา asthma อิงจาก

1.       Severity (ในผู้ป่วยที่ “ยังไม่ได้” on daily therapy)

2.       Control (ในผู้ป่วยที่ “กำลัง” on daily controller therapy)

  • เป้าหมายคือ ลด impairment (อาการ/ข้อจำกัดกิจกรรม) และลด future risk (exacerbations, oral glucocorticoid bursts, hospitalization ฯลฯ)

1) ASSESSMENT OF SEVERITY (ผู้ป่วยยังไม่ on daily therapy)

นิยาม

  • Asthma severity = intrinsic intensity of disease
  • ประเมินจาก 2 domain:
    • Impairment (ช่วง 2–4 สัปดาห์ล่าสุด)
    • Risk (โดยเฉพาะจำนวน exacerbations ใน 12 เดือนที่ต้องใช้ oral glucocorticoids)
  • จัดระดับ severity โดยเอา “หมวดที่รุนแรงที่สุด” ที่พบ (worst category wins)
  • Severity ไม่ได้ทำนายความรุนแรงของแต่ละครั้งของ exacerbation (mild asthma ก็เกิด severe exacerbation ได้)

องค์ประกอบที่ใช้ประเมิน

Impairment

  • Frequency ของ symptoms
  • Nighttime awakenings
  • SABA use เพื่อ symptom relief (ไม่รวม prophylaxis ก่อน exercise)
  • Activity limitation
  • Spirometry (ถ้าทำได้)

Risk

  • จำนวนครั้งที่ต้อง oral glucocorticoids ในปีที่ผ่านมา (+ดูความรุนแรง/ความใกล้ล่าสุดประกอบ)

เมื่อไรควรประเมิน

  • หลัง confirm diagnosis หรือครั้งแรกที่เจอผู้ป่วย (ก่อนเริ่ม controller)
  • ถ้าพบตอนมาด้วย exacerbation ให้ถามอาการ/การใช้ SABA “ก่อนเริ่มกำเริบครั้งนี้” เพื่อประเมิน baseline impairment

2) INITIATION OF THERAPY (เริ่ม controller อย่างไรโดยหลักคิด)

  • ระดับ severity ขณะ “ยังไม่ใช้ controller” → กำหนด step ของ initial therapy
  • เด็กเล็กมาก (โดยเฉพาะ <5 ปี) ต้องคำนึงถึง risk of persistent asthma และ phenotype (เช่น recurrent virus-induced wheezing) ร่วมด้วย
  • รายละเอียดเลือกยาราย step (ICS dose, add-on ฯลฯ) ไปอยู่ในหัวข้อ “controller therapies” แยกต่างหาก แต่หลักการคือ:
    • Persistent asthma → ควรมี daily controller therapy (มักเริ่มด้วย inhaled glucocorticoids)

3) SEVERITY ในผู้ป่วยที่ “on daily therapy”

  • ในคนที่ใช้ controller อยู่แล้ว ประเมิน “control” สำคัญกว่า severity
  • แนวคิดใหม่: severity สะท้อน “ความยากในการทำให้คุมได้” (difficulty achieving control)
    • เช่น คุมได้แต่ต้องใช้ 3 controllers ถึงจะคุมอยู่ → ถือว่า severe asthma ได้
  • Severity ถูก influence โดย
    • phenotype
    • environmental/social factors (smoke, PM2.5/pollution, stress/violence)
    • adherence + inhaler technique
    • comorbidities

4) ASSESSMENT OF CONTROL (หัวใจของการติดตาม)

นิยาม (NAEPP)

  • Asthma control = ระดับที่ therapy ลด/กำจัด manifestations ของ asthma
  • ประเมินทั้ง
    • impairment + risk
    • และ treatment adverse effects

Persistent symptoms (impairment) = risk factor ต่อ severe exacerbations (risk) แม้ predictors ไม่เหมือนกันทั้งหมด

ประเด็นใน history ที่ควรถามทุก follow-up

  • Medications: ชื่อยา/ขนาด/ความสม่ำเสมอ (adherence), adverse effects
  • Medical utilization (ED/urgent care/hospital, oral steroid bursts)
  • School attendance/performance
  • Physical activity limitation
  • Psychosocial factors

Standardized questionnaires (ช่วยได้มากในคลินิก)

  • ACT / Childhood ACT (อายุ 4–11 ปี)
  • TRACK (preschool) — มีส่วนประเมิน risk ผ่าน oral glucocorticoid use ใน 12 เดือน
  • PACCI
  • Asthma APGAR (Activities, Persistent triGgers, Asthma meds, Response) เน้น “actionable items” เช่น trigger, adherence, technique

Objective testing

  • Spirometry (ทำได้ตั้งแต่อายุ ~5 ปี) ใช้ร่วมกับอาการ/การใช้ยา
  • FeNO เป็น adjunct ในรายที่ยัง uncertain หลัง history+exam+spirometry

Modifiable factors ที่พบบ่อยใน poor control

  • Underuse of controller medications (พบบ่อยมาก)
  • Trigger exposure (animal dander, dust mite) → ต้องทำ environmental control
  • Secondhand cigarette smoke / vaping
  • caregiver beliefs/expectations ต่ำต่อประโยชน์ของ controller
  • household competing priorities (งาน/เงิน/ความปลอดภัย/ปัญหาครอบครัว ฯลฯ)

5) MONITORING & DOSE ADJUSTMENT

Timing ของการ re-evaluate หลังเริ่ม controller

  • Moderate–severe persistent asthma: 2–4 สัปดาห์
  • Mild persistent asthma: 4–6 สัปดาห์
    (เพราะโดยทั่วไปต้อง 2–6 สัปดาห์จึงจะเห็น response ต่อ intervention)

ความถี่ follow-up หลังจากนั้น (อิง control)

  • Well-controlled: โดยมากทุก ~3 เดือน (ปรับตาม severity/บริบท)
  • Not well-controlled: follow-up 2–6 สัปดาห์
  • Very poorly controlled: follow-up ~2 สัปดาห์ หลัง step-up

Step-up therapy

  • Escalate ได้ทุกเวลาเมื่อ control ไม่ดี
  • ต้องเช็ค adherence + technique + trigger + comorbidities ก่อน เพิ่มยา
  • ต้องคุยเรื่อง adverse effects สำคัญ เช่น
    • montelukast: neuropsychiatric/behavioral symptoms
    • high-dose inhaled glucocorticoids: growth effects/adrenal suppression (โดยเฉพาะถ้า dose สูงนาน)

Step-down therapy

  • พิจารณาเมื่อคุมได้ อย่างน้อย 3 เดือน
  • ปรับตาม: baseline severity, ความยากในการคุม, trigger burden
  • มักลองลดช่วง summer; ระวังช่วงเปิดเทอม/ฤดู viral season
  • ถ้ามี ongoing exposures (smoke/เลี่ยงสัตว์ไม่ได้ในบ้าน/รร./daycare) → ลดช้าลง
  • หลังลด ให้ reassess ใน 1–2 เดือน และพยายามลดให้เหลือ regimen ต่ำสุดที่ยังคุมได้

Acute exacerbations ระหว่างติดตาม

  • ต้อง intensify management ทุกเมื่อ รวมถึง oral glucocorticoids ตามข้อบ่งชี้ และกลับไปใช้แผน acute exacerbation pathway

Practical takeaways (เอาไปใช้ในคลินิก)

  • ใหม่/ยังไม่ on controller → classify severity (impairment+risk) → เลือก initial step
  • on controller แล้ว → assess control + หา modifiable causes ก่อน step-up
  • Follow-up interval สั้นลงตามความคุมไม่ได้
  • Step-down เมื่อคุมได้ ≥3 เดือน และต้องติดตามใกล้ชิดหลังลด

Long-term controller therapy สำหรับ persistent asthma ในเด็ก <12 ปี

GOALS

  • คุมอาการ + ลด exacerbations ด้วยยาน้อยที่สุด และ side effects น้อยที่สุด
  • เน้น “control” มากกว่า “severity” เมื่อผู้ป่วย on controller แล้ว
    (แต่ก่อนเริ่มยาให้ classify severity เพื่อเลือก initial step)

CONTROLLER OPTIONS (ภาพรวม)

หลัก ๆ ที่ใช้ในเด็ก <12 ปี

1.       Inhaled glucocorticoids (ICS) = first-line controller

2.       ICS + LABA (เฉพาะร่วมกับ ICS เท่านั้น; ห้าม LABA monotherapy)

3.       Leukotriene receptor antagonist (LTRA) (เช่น montelukast) = alternative/add-on บางราย

4.       LAMA (tiotropium) ≥6 ปี = add-on ใน severe symptomatic asthma ที่คุมไม่อยู่

5.       Biologics ≥6 ปี (เช่น anti-IgE, anti-IL-4R, anti-IL-5/5R) = step-up ใน severe asthma phenotype เหมาะสม

Rarely used as daily control

  • chronic oral glucocorticoids, chromones, theophylline (เพราะ benefit/harms ไม่คุ้ม + monitoring/side effects)

1) ICS (Inhaled glucocorticoids) – “ตัวหลัก”

ทำไมเป็น first-line

  • anti-inflammatory ที่ effective ที่สุดใน asthma
  • ลดทั้ง impairment (symptoms, activity limitation) และ risk (exacerbations, decline of lung function)
  • dose-response ค่อนข้าง “flat” → benefit ส่วนใหญ่ได้ที่ low dose, เพิ่ม dose ได้ประโยชน์เพิ่มไม่มากแต่ side effects เพิ่ม

Time course (จำให้แม่น)

  • symptom improvement อาจเร็ว, inflammation ลดได้ภายในชั่วโมง
  • lung function ดีขึ้นต่อเนื่อง ~4 สัปดาห์แล้ว plateau
  • อาจใช้ถึง ~3 เดือนกว่าจะ “นิ่ง” → การปรับ dose ระยะยาวมักเว้นช่วงพอสมควร
  • หยุดยา → control ค่อย ๆ กลับแย่ลง (benefit มัก “อยู่เฉพาะตอนใช้”)

จุดสำคัญด้าน “disease modification”

  • ICS สำคัญต่อการคุมโรค แต่ ไม่เปลี่ยน natural history หลังหยุดยา (โดยรวม)

Adverse effects (ภาพรวม)

  • ผลต่อ adult height โดยรวม “เล็กน้อย”
  • เน้นลด risk ด้วย “lowest effective dose” + device/technique ที่ถูกต้อง

2) LTRA (เช่น montelukast) – ทางเลือก/เสริม

บทบาท

  • less effective than ICS สำหรับคนส่วนใหญ่ แต่บางรายตอบสนองดี
  • ใช้ได้เป็น
    • alternative ใน mild persistent (โดยเฉพาะ adherence/technique inhaler แย่)
    • add-on ในบางราย (แต่ทายล่วงหน้าไม่ได้ว่าใครจะ respond)

Safety note สำคัญ

  • boxed warning: serious neuropsychiatric events (รวม suicidal thoughts/actions) → ต้อง counsel ให้ชัดเจน + เฝ้าระวัง

3) LABA – ใช้ “ร่วมกับ ICS เท่านั้น”

  • ช่วยคุมอาการ/กลางคืน/กัน exercise-induced bronchoconstriction
  • ห้าม LABA monotherapy (ไม่ครอบคลุม inflammation + safety)
  • formoterol = fast onset (ใกล้ albuterol)
    salmeterol/vilanterol = slower onset
  • เด็ก: evidence โดยรวมชี้ว่า add LABA → lung function ดีขึ้น + ลด rescue use; ผลต่อ exacerbations อาจไม่เด่นกว่าเพิ่ม ICS ทุก outcome
    ดังนั้น “ไม่คุม → add LABA” มักดีกว่า “เพิ่ม ICS dose” แต่ต้องยืดหยุ่นตามราย

4) LAMA (tiotropium) ≥6 ปี

  • add-on ใน severe symptomatic asthma ที่คุมไม่อยู่บน ICS + maintenance อื่น ๆ
  • ไม่ใช่ initial therapy และไม่ใช่ sole therapy
  • เพิ่ม lung function/อาจลด exacerbations บางการศึกษา; patient-important outcomes (symptoms/rescue use) อาจไม่ต่างชัด

5) Biologics ≥6 ปี (เลือกตาม phenotype)

ใช้ใน severe asthma ที่คุมไม่อยู่แม้ optimize standard therapy

  • Omalizumab (anti-IgE): ต้องมี allergic sensitization ต่อ perennial aeroallergen + IgE/weight อยู่ในเกณฑ์ dosing
  • Dupilumab (anti-IL-4Rα): moderate-to-severe asthma (มัก eosinophilic/type 2); มีประโยชน์ในคนที่มี atopic dermatitis ร่วม (ใช้ dosing ตาม AD ได้)
  • Mepolizumab / Benralizumab (anti-IL-5/5R): severe eosinophilic asthma (ดู blood eosinophils ± FeNO/อื่น ๆ ตามแนวทางสถานที่)

INITIATING CONTROLLER THERAPY (สรุปแบบใช้งานจริง)

A) Mild persistent asthma (Step 2)

Preferred: low-dose daily ICS
Alternatives (เลือกกรณี):

  • intermittent low-dose ICS ให้ “เมื่อใช้ SABA” (symptom-based) บาง guideline/บางบริบท
    แต่โดยหลัก: ยังแนะนำ daily มากกว่า intermittent ในเด็ก; ถ้าล้มเหลว (ต้อง unscheduled visit หรือ oral steroids) → กลับไป daily
  • daily LTRA (ถ้า inhaler adherence/technique เป็นปัญหาหนัก) แต่ต้องชั่ง neuropsychiatric risk

B) Moderate persistent asthma (โดยแนวคิด Step 3)

  • โดยทั่วไป “ไม่คุมด้วย low-dose ICS” → add LABA (โดยเฉพาะถ้าใช้ fast-onset LABA เช่น formoterol ในบาง regimen)
  • ทางเลือก: medium-dose ICS หรือ ICS + LTRA (เลือกตาม phenotype/ความร่วมมือ/side effects)

C) Severe persistent asthma (Step 4–5+)

  • มักต้อง medium/high-dose ICS + LABA (ตามอายุ/step)
  • ถ้ายังคุมไม่อยู่ (≥6 ปี): เพิ่ม tiotropium → ถ้ายังไม่คุม พิจารณา biologics ตาม phenotype
  • chronic oral glucocorticoids ควรเป็นทางเลือกสุดท้ายจริง ๆ (เพราะ toxicity สูง) และปัจจุบันมักหลีกเลี่ยงได้มากขึ้นด้วย LAMA/biologics

STEP-UP (หลักการสั้น ๆ แต่สำคัญมาก)

ก่อน step-up ทุกครั้งต้องเช็ค:

1.       Adherence (underuse ของ controller พบบ่อยสุด)

2.       Inhaler technique / device match age

3.       Trigger exposure (smoke/vaping, dust mite, pet dander ฯลฯ)

4.       Comorbidities (allergic rhinitis/sinusitis, GERD, OSA ฯลฯ)

5.       Confirm diagnosis/consider alternative if atypical

วิธี step-up:

  • Not well-controlled → ขยับ 1 step
  • Very poorly controlled → ขยับ 2 steps ± short course oral glucocorticoids
  • Reassess โดยทั่วไป 4–6 สัปดาห์
  • ถ้าไม่ตอบสนองต่อ option หนึ่งใน step เดียวกัน → ลอง “อีก option ใน step เดียวกัน” ได้ก่อนขึ้น step (ถ้าไม่วิกฤต)

STEP-DOWN

  • พิจารณาเมื่อ คุมได้ ≥3 เดือน
  • ลดไปสู่ lowest effective ICS dose/regimen
  • หลังลดควร reassess ใน 1–2 เดือน (โดยเฉพาะเด็กที่มี trigger เลี่ยงไม่ได้/เคย severe)

MINIMIZING ICS ADVERSE EFFECTS (ทำได้ทันทีในคลินิก)

  • ใช้ lowest effective dose
  • ใช้ MDI + spacer/valved holding chamber (≤6 ปีมักต้อง facemask)
  • optimize trigger control + รักษา comorbidities
  • สอนและ “ตรวจ technique ทุกครั้ง” (อย่าคิดว่าเคยสอนแล้วจะถูกเสมอ)

COVID-19 (key message)

  • ผู้ป่วย asthma ควร continue regular asthma meds เพื่อคุมโรคให้ดีที่สุด รวมถึง ICS / oral glucocorticoids (เมื่อมีข้อบ่งชี้) / biologics ตามเดิม เพราะ baseline poor control = risk สำคัญของ outcome แย่จาก viral illnesses
