Fever management

Fever in adult

🔹 Definition (เข้าใจให้ถูกก่อน)

  • Fever = elevation ของ core temp จาก set-point ที่ hypothalamus “ถูก reset สูงขึ้น”
  • โดยทั่วไปใช้ cutoff:
    • 37.8–38°C (practical)
    • หรือ >37.2°C (เช้า) / >37.7°C (บ่าย)

🌡️ Normal Physiology

  • Normal temp: 35.3–37.7°C (mean ~36.7°C)
  • Circadian variation:
    • เช้าต่ำ / เย็นสูง (↑ ~0.5°C)
  • Elderly → baseline temp ต่ำ → infection อาจไม่มีไข้

🔥 Fever vs Hyperthermia (ต้องแยกให้ชัด)

Feature

Fever

Hyperthermia

Set-point

↑ (hypothalamus reset)

ปกติ

Cause

cytokine-mediated

heat overload

Antipyretic

✔ ได้ผล

❌ ไม่ได้ผล

Treatment

antipyretic

cooling เท่านั้น

👉 Hyperthermia = medical emergency

🔬 Pathophysiology (High-yield)

🔹 Step 1: Exogenous pyrogen

  • bacteria (LPS)
  • toxin (TSST-1)
  • virus, fungi

🔹 Step 2: Cytokine release

  • IL-1
  • IL-6 (สำคัญสุด clinically)
  • TNF-α

👉 จาก macrophage / neutrophil

🔹 Step 3: Hypothalamic activation

  • cytokine → ↑ PGE2
  • → ↑ hypothalamic set-point

🔹 Step 4: Heat generation

  • vasoconstriction → รู้สึกหนาว
  • shivering / thermogenesis
  • behavior (ใส่เสื้อเพิ่ม)

🔹 Step 5: Defervescence

  • set-point ↓
  • vasodilation + sweating

👉 กลไกเหมือน thermostat

🧠 Key Clinical Implications

🔹 1. ทำไมคนไข้ “หนาว” ตอนเริ่มไข้?

→ set-point ↑ → body “คิดว่าหนาว”

🔹 2. ทำไมเหงื่อออกตอนไข้ลง?

→ set-point ↓ → heat loss

🔹 3. Cytokine สำคัญ

  • IL-6 = main driver fever

🔹 4. CNS fever

  • hemorrhage / tumor → hyperpyrexia

⚠️ Hyperpyrexia

  • 41.5°C
  • cause:
    • severe infection
    • CNS hemorrhage
      👉 ต้องรีบรักษา

💊 Antipyretic Mechanism

🔹 NSAIDs

  • inhibit COX → ↓ PGE2

🔹 Acetaminophen

  • central COX inhibition

🔹 Steroid

  • ↓ cytokine + ↓ PGE2

🧠 Clinical Use of Fever

🔹 Benefit

  • ↓ symptom (myalgia, headache)
  • ↓ O2 demand

👉 แต่ ไม่มีหลักฐานชัดว่าลด mortality

🔹 Harm (theoretical)

  • อาจลด immune response
  • mask diagnosis

🧠 When to Treat Fever

✅ ควรให้

  • symptomatic
  • cardiac / pulmonary disease
  • hyperpyrexia
  • CNS disease

❌ อาจไม่จำเป็น

  • stable patient
  • ต้องการดู fever pattern

💊 Drug Choice (practical)

🔹 First-line

  • Acetaminophen
    • 325–650 mg q4–6h
    • max 4 g/day

🔹 Alternative

  • NSAIDs (ibuprofen, naproxen)
    👉 ระวัง:
  • AKI
  • GI bleed

❄️ Cooling

  • ใช้ใน hyperpyrexia
  • ต้องใช้ร่วมกับ antipyretic
  • หลีกเลี่ยง overcooling → vasoconstriction

⚠️ Important Clinical Clues

🔹 Relative bradycardia

→ think:

  • typhoid
  • brucellosis
  • drug fever

🔹 No fever in severe infection

พบใน:

  • elderly
  • steroid use
  • CKD
  • sepsis (บางราย hypothermia)

🔹 Fever pattern (ใช้ได้น้อย)

  • malaria → tertian/quartan
  • lymphoma → Pel-Ebstein (ไม่ reliable)

🔥 High-Yield Summary

  • Fever = set-point ↑ via cytokine → PGE2
  • IL-6 = key mediator
  • Fever ≠ hyperthermia
  • Antipyretic → ↓ PGE2
  • Treat when symptomatic / high-risk
  • Hyperthermia → cooling only
  • Elderly → infection without fever ได้

Fever in Children

🔹 Definition (ตามอายุสำคัญมาก)

👶 <3 เดือน

  • Rectal ≥38.0°C = serious until proven otherwise

👶 3–36 เดือน

  • ≥38.0–39.0°C = fever
  • ≥39.0°C (no source) → higher risk SBI

👦 เด็กโต

  • Oral ≥37.8–39.4°C

👉 “Height of fever” สำคัญน้อยกว่า appearance

🌡️ Measurement (high-yield)

Method

Note

Rectal

gold standard

Oral

ต่ำกว่า rectal ~0.6°C

Axillary

unreliable

Tympanic

acceptable

Forehead

ไม่ควรใช้ตัดสิน clinical

👉 Neonate → ใช้ rectal เท่านั้น

🔬 Pathophysiology (เหมือนผู้ใหญ่)

  • Infection → cytokine (IL-1, IL-6, TNF)
  • → ↑ PGE2
  • → ↑ hypothalamic set-point
  • → vasoconstriction + heat production

⚠️ Fever vs Hyperthermia

Feature

Fever

Hyperthermia

Set-point

↑

ปกติ

Cause

cytokine

heat/drug

Antipyretic

✔

❌

Risk

moderate

fatal

👉 Hyperthermia: hot dry skin + CNS dysfunction

🧠 Clinical Meaning

🔹 Fever = physiologic response

  • ไม่ใช่ disease
  • เป็นส่วนหนึ่งของ immune response

🔹 Benefit

  • ↓ bacterial growth
  • ↑ immune activity

🔹 Harm

  • ↑ O2 consumption
  • ↑ metabolic demand
    👉 สำคัญใน:
  • shock
  • cardiac disease
  • dehydration

🧠 Evaluation (clinical approach)

🔹 Step 1: Look appearance (สำคัญสุด)

  • toxic?
  • lethargy?
  • poor feeding?

👉 appearance > temperature

🔹 Step 2: Duration

  • 5 วัน → ↑ risk SBI

🔹 Step 3: Associated symptoms

  • meningismus
  • rash (petechiae)
  • respiratory distress

🔹 Step 4: Risk group

  • neonate
  • immunocompromised
  • chronic disease

💊 Management Principle

🔹 Goal = “comfort” ไม่ใช่ normalize temp

🔹 Indication for treatment

  • discomfort
  • dehydration
  • cardiopulmonary disease
  • shock
  • temp ≥40°C

🔹 NOT routine

  • healthy child → ไม่จำเป็นต้องลดไข้เสมอ

💊 Antipyretic

🔹 First-line: Acetaminophen

  • 10–15 mg/kg/dose q4–6h
  • max 75 mg/kg/day

🔹 Alternative: Ibuprofen (>6 เดือน)

  • q6–8h
  • max 40 mg/kg/day

❌ Avoid

  • aspirin → Reye syndrome
  • alternating/combination → dosing error

⚠️ Important Points

🔹 Response to antipyretic

❌ ไม่ช่วยแยก viral vs bacterial

🔹 Treat fever ≠ improve outcome

  • ไม่มี evidence ลด mortality

🔹 Fever does NOT cause brain damage

  • แม้ >40°C (ใน fever)

🔹 Neonate rule

  • <3 เดือน → evaluate before giving drug

❄️ Cooling

🔹 ไม่แนะนำ routine

  • tepid sponge → discomfort ↑

🔹 ใช้เฉพาะ:

  • hyperthermia
  • severe fever needing rapid control

🚨 Red Flags (ต้องรีบ evaluate)

  • age <3 เดือน
  • toxic appearance
  • altered mental status
  • stiff neck
  • petechiae/purpura
  • dehydration
  • fever >5 วัน
  • worsening fever

🔥 Clinical Pearls

  • Fever = immune response ไม่ใช่ศัตรู
  • treat “patient” ไม่ใช่ “ตัวเลข”
  • appearance > temperature
  • antipyretic = improve comfort
  • hyperthermia = emergency
  • neonate fever = sepsis until proven otherwise
