Giant Papillary Conjunctivitis (GPC)

🔹 Overview

  • GPC = noninfectious inflammatory conjunctivitis
  • เกิดจาก mechanical irritation + immune reaction
  • สัมพันธ์มากกับ contact lens

👉 ถือเป็น ocular allergy subtype (แต่ ไม่ใช่ allergen-driven โดยตรง)

📊 Epidemiology

  • พบใน soft contact lens > rigid lens
  • ~1–5% ของ soft lens users
  • พบมากใน young adults

🧬 Pathophysiology

  • Debris บน lens → antigen
  • mechanical friction จาก blinking
    → immune response (Type I + IV)

👉 ทำให้เกิด:

  • mast cell / eosinophil activation
  • papillary hypertrophy

🔍 Clinical Features

🔹 Symptoms

  • Itching
  • Foreign body sensation
  • Mucus discharge (ตอนเช้าเด่น)
  • Blurred vision
  • Contact lens intolerance

👉 severe → ใส่ lens ไม่ได้เลย

🔹 Signs (key diagnosis)

  • Giant papillae (>1 mm) บน upper tarsus
  • Conjunctival hyperemia
  • Mucus strands
  • Eyelid thickening ± ptosis

🧠 Diagnosis

👉 Clinical + everting upper eyelid

  • Hx: contact lens intolerance
  • PE: giant papillae

❌ ไม่ต้อง allergy test

📈 Staging (high-yield concept)

  • Stage I → mild itching, no papillae
  • Stage II → papillae เริ่มเกิด
  • Stage III → mucus มาก + lens movement
  • Stage IV → severe → lens intolerance

💊 Management (สำคัญมาก)

🟥 Core principle

👉 Remove offending cause

🟢 Step 1: Non-pharmacologic

  • ❌ หยุด contact lens 2–4 สัปดาห์
  • เปลี่ยน lens / cleaning system
  • ลด wearing time
  • ใช้ preservative-free solution

🟡 Step 2: Medical therapy

  • Artificial tears
  • Topical antihistamine / mast cell stabilizer
    • เช่น olopatadine, ketotifen

🔴 Step 3: Severe cases

  • Topical steroid (short course)
    • เช่น loteprednol

👉 ใช้ภายใต้ ophthalmologist

❌ What NOT useful

  • ❌ Allergen avoidance (ไม่ใช่ allergen-driven)
  • ❌ Immunotherapy

🔁 Prognosis

  • ดีมาก
  • อาการดีขึ้นใน <1 สัปดาห์ หลังหยุด lens
  • ส่วนใหญ่กลับมาใส่ lens ได้

🧠 Clinical Pearls

  • Contact lens intolerance = think GPC
  • Giant papillae = diagnostic clue
  • Treatment = remove trigger > drug
  • Soft lens > risk สูง
  • ไม่ใช่ allergic conjunctivitis ธรรมดา
