Orbital Fracture

🔑 Key concept

  • มักเกิดจาก high-impact trauma (assault, MVC)
  • ต้องคิดเสมอว่า มี associated injury (intracranial + globe injury)
  • Priority = rule out vision-threatening & life-threatening conditions

🧩 1. Mechanism & Epidemiology

  • ผู้ป่วยหลัก: ชายวัยรุ่น–วัยทำงาน
  • กลไก:
    • Assault (adult)
    • MVC
    • Sports / fall (เด็ก)

🧠 2. Important anatomy (high-yield)

  • Floor (maxillary bone) → weakest → fracture บ่อย
  • Medial wall (ethmoid) → thin
  • Infraorbital nerve → sensory cheek/upper lip
  • EOM → เสี่ยง entrapment

🦴 3. Types of orbital fracture

🔹 1. Orbital floor fracture (blowout) ⭐

  • กลไก: blunt object (เช่น ลูกบอล)
  • complication:
    • Inferior rectus entrapment
    • Enophthalmos
    • infraorbital nerve injury

🔹 2. Zygomatic fracture

  • จากแรงกระแทกด้านข้าง
  • มักร่วมกับ floor fracture

🔹 3. Medial wall fracture

  • เสี่ยง:
    • medial rectus entrapment
    • lacrimal system injury

🔹 4. Orbital roof fracture

  • พบบ่อยในเด็ก
  • ⚠️ สัมพันธ์กับ intracranial injury สูง

🚨 4. Red flags (ต้องรีบแยก)

Vision-threatening

  • Ruptured globe
  • Orbital compartment syndrome
  • Optic nerve injury
  • Retinal detachment
  • Hyphema

👉 ถ้าสงสัย → consult ด่วน / treat ทันที


👁️ 5. Clinical features

Symptoms

  • Pain / swelling
  • Diplopia (โดยเฉพาะ upward gaze)
  • Blurred vision
  • Facial numbness

Signs

  • Periorbital ecchymosis
  • Step-off deformity
  • Crepitus (orbital emphysema)
  • ↓ sensation infraorbital/supraorbital nerve
  • Limited EOM / pain with movement

⚠️ Special clue

  • N/V + bradycardia → oculocardiac reflex → suspect muscle entrapment

🔍 6. Diagnosis

✔️ Suspect when:

  • Facial trauma +
    • Diplopia
    • ↓ vision
    • Numbness face
    • Step-off deformity
    • Proptosis / enophthalmos

🖥️ Imaging (Gold standard)

  • CT orbit (thin-cut ≤1 mm) ⭐
  • ดู:
    • fracture
    • muscle entrapment
    • fat herniation

👉 X-ray sensitivity ต่ำ (~50%)


🧪 7. Evaluation (ใช้จริงใน ER)

ต้องทำทันที:

  • Visual acuity ⭐
  • Pupillary reflex (RAPD)
  • EOM
  • Fundoscopy
  • Slit lamp (ถ้าได้)

⚠️ ห้ามกดลูกตา (ถ้าสงสัย globe rupture)


🧠 8. Initial management

🚑 Step 1: Primary survey

  • ABC + cervical spine

🚨 Step 2: Rule out emergency (แน่น แตก ดับ)

  • Orbital compartment syndrome → lateral canthotomy ทันที
  • Globe rupture → shield eye
  • Optic neuropathy

📞 Step 3: Consult ophthalmology ถ้ามี

  • ↓ vision
  • EOM limitation
  • proptosis
  • medial canthus injury
  • suspected entrapment

🏥 9. Indications for surgery

Urgent (<24 hr)

  • Muscle entrapment (esp. children)
  • Severe diplopia
  • Oculocardiac reflex

Elective

  • Enophthalmos
  • Cosmetic deformity
  • Persistent diplopia

👉 early surgery → better outcome


🏡 10. Discharge advice (ถ้าไม่ admit)

  • Cold compress 48 hr
  • Elevate head
  • ❌ ห้ามสั่งน้ำมูก
  • Follow-up ≤24 hr

❓ 11. Controversial treatments

  • Steroid → อาจช่วย diplopia แต่ evidence limited
  • Antibiotic prophylaxis → ไม่จำเป็น routine

🧠 Clinical pearls

  • 🔴 Orbital fracture ≠ isolated injury → หา intracranial/globe injury เสมอ
  • 🔴 Diplopia + upward gaze → think inferior rectus entrapment
  • 🔴 เด็ก + N/V + bradycardia → trapdoor fracture
  • 🔴 CT = investigation of choice
  • 🔴 Orbital compartment syndrome → don’t wait CT → decompress immediately

📌 Practical takeaway

  • Trauma eye → check vision first
  • CT orbit → confirm
  • Rule out emergency → consult early
  • Entrapment → surgery early
  • Mild → conservative + close follow-up
