Cardiac Arrest Prognosis Profile III (Intra-arrest Findings)

ไม่ได้คงที่ แต่สามารถ update ได้จากข้อมูลที่เกิดขึ้นระหว่าง
resuscitation
ข้อมูลเหล่านี้ประกอบด้วย
rhythm evolution, ETCO₂, POCUS
cardiac activity, blood gas/POCT, lactate trend และ response
ต่อ resuscitation
หลักสำคัญคือ
ไม่มี intra-arrest finding ใดเพียงตัวเดียวที่มีความแม่นยำเพียงพอสำหรับการตัดสินใจหยุด CPR
แต่เมื่อ independent markers หลายตัวชี้ไปในทิศทางเดียวกัน ความมั่นใจในการประเมิน prognosis จะเพิ่มขึ้น
1. Initial rhythm และ rhythm
evolution
โดยทั่วไป prognosis เรียงจาก
VF/pVT > PEA > asystole
แต่ rhythm ไม่ควรถูกมองแบบ
static
ตัวอย่างเช่น
Persistent asystole
มี prognosis แย่กว่า
PEA →
VF/pVT
หรือ
PEA →
transient ROSC
การเกิด shockable rhythm หรือ
transient circulation ระหว่าง resuscitation แสดงว่าหัวใจยังมี physiological responsiveness
ในทางตรงกันข้าม persistent asystole ตลอด prolonged high-quality CPR เป็น adverse
prognostic sign ที่สำคัญ
2. ETCO₂
ETCO₂ เป็นหนึ่งใน bedside
physiologic markers ที่มีข้อมูลมากที่สุดระหว่าง CPR
เนื่องจากเมื่อ ventilation คงที่ ปริมาณ CO₂ ที่มาถึงปอดขึ้นกับ pulmonary
blood flow จึงสามารถใช้เป็น indirect marker ของ
cardiac output จาก chest compression ได้
โดยทั่วไป
ETCO₂ สูงขึ้น → pulmonary
blood flow/CPR effectiveness ดีกว่า
และ sudden sustained rise ของ ETCO₂ อาจเป็นสัญญาณของ ROSC
Classic studies พบว่าผู้ที่เกิด ROSC มี ETCO₂ สูงกว่าผู้ที่ไม่ ROSC อย่างมาก
โดยเฉพาะ
Persistent ETCO₂ <10 mmHg หลัง CPR
ประมาณ 20 นาที
สัมพันธ์กับ extremely poor prognosis
แต่ subsequent studies พบว่า
cutoff นี้ไม่ได้มี specificity 100%
ดังนั้น ETCO₂ <10 ไม่ควรถูกใช้เป็นเหตุผลเดียวในการ terminate resuscitation
ต้องตรวจสอบด้วยว่า
- airway/ET
tube ถูกต้อง
- ventilation
ไม่มากเกินไป
- chest
compression มีคุณภาพ
- ไม่มี massive PE หรือ pulmonary
blood-flow obstruction
- ไม่มี technical problem
3. POCUS cardiac activity
POCUS สามารถแยก PEA ออกเป็นสอง
physiological phenotypes
Pseudo-PEA: มี organized cardiac
contraction แต่คลำ pulse ไม่ได้
กับ
True PEA/cardiac standstill: ไม่มี meaningful
myocardial movement
การเห็น cardiac activity ระหว่าง
pulse check สัมพันธ์กับ ROSC และ survival
ที่สูงกว่า cardiac standstill อย่างชัดเจน
ดังนั้น
PEA + cardiac motion
มี prognosis ดีกว่า
PEA + complete cardiac standstill
แต่ cardiac standstill ไม่ได้มี
specificity 100% สำหรับ death จึงไม่ควรใช้
ultrasound เพียงอย่างเดียวเพื่อหยุด CPR
POCUS มีประโยชน์เพิ่มเติมในการค้นหา reversible
cause เช่น
tamponade, RV strain/possible PE, severe hypovolemia และ pneumothorax
โดยต้องทำโดยไม่ทำให้ chest compression
interruption ยาวขึ้น
4. pH / Blood gas (adjusted OR for no-ROSC 0.03 ต่อการเพิ่ม pH 1 unit)
Severe acidosis ระหว่าง CPR สัมพันธ์กับ probability of ROSC และ survival
ที่ลดลง
pH สะท้อนทั้ง
- duration
of arrest
- adequacy
of perfusion
- ventilation
- underlying
metabolic state
ดังนั้น
progressively falling pH despite adequate CPR
เป็น adverse sign
แต่ absolute pH ไม่ควรเป็น
TOR cutoff
ผู้ป่วยบางกลุ่ม เช่น poisoning,
hyperkalemia หรือ hypothermia สามารถมี severe
acidosis และยัง recover ได้หาก underlying
cause reversible
จึงควรถามเสมอว่า
pH ต่ำเพราะ prolonged irreversible
arrest หรือเป็นส่วนหนึ่งของ reversible pathology?
5. Potassium (OR 2.28)
Hyperkalemia ระหว่าง prolonged arrest อาจเกิดจาก cellular injury และ severe
acidosis จึงสามารถเป็น marker ของ prolonged
ischemia ได้
แต่ในอีกบริบทหนึ่ง hyperkalemia อาจเป็น สาเหตุของ arrest
ตัวอย่าง
ESRD + missed dialysis + K 8.5 + PEA
ควรตีความต่างจาก
prolonged unwitnessed arrest + K 8.5 หลัง CPR
45 นาที
รายแรกเป็น potentially reversible

systemic ischemia

clinical context และ timing ของ blood

6. Lactate
Initial lactate สูงสัมพันธ์กับ poor
outcome ในหลาย studies แต่ discrimination
ของ single value ไม่ดีพอสำหรับใช้เดี่ยว ๆ
Trend มีประโยชน์มากกว่า
Favorable pattern
Lactate stable/decreasing + pH improving
Unfavorable pattern
Lactate progressively rising + pH progressively falling
การที่ lactate ลดลงระหว่าง
ongoing CPR อาจสะท้อน effective circulation และ metabolic recovery (เจาะ POCT
ซ้ำทุก 10 นาที: ทุก lactate
ที่ลดลง 1 mmol/L สัมพันธ์กับ sustained
ROSC เพิ่มขึ้นประมาณ 16% (aOR 1.16))
อย่างไรก็ตาม lactate ยังขึ้นกับ
pre-arrest shock, sepsis, seizures, epinephrine และ hepatic
clearance จึงต้องตีความร่วมกับบริบท
7. Response to resuscitation
บางครั้ง response ต่อ intervention
มีข้อมูลทาง prognosis มากกว่าค่า laboratory

persistently very low + progressive metabolic deterioration
หลัง high-quality CPR เป็นเวลานาน
เป็น convergence ของ adverse prognostic markers
Dynamic Cardiac Arrest Prognosis Profile
การประเมินระหว่าง CPR จึงไม่ควรถามว่า
“มี marker ตัวไหนที่บอกให้หยุด?”
แต่ควรถามว่า

cause เช่น hypothermia, poisoning, PE หรือ correctable metabolic disturbance

การประเมิน cardiac arrest ที่เหมาะสมจึงควรประกอบด้วยสามชั้น
1. Baseline prognosis
Functional status + age + frailty + comorbidities

Cardiac arrest prognosis should be multimodal, dynamic,
and cause-specific.
ไม่ควรมี age, diagnosis, laboratory
value, ETCO₂, POCUS finding หรือ CPR duration ตัวใดตัวหนึ่ง ทำหน้าที่เป็น absolute
futility criterion
การที่ baseline prognosis + arrest
characteristics + presumed cause + intra-arrest physiology หลายตัวซึ่งเป็นอิสระต่อกันชี้ไปในทิศทางเดียวกัน
จึงเป็นข้อมูลที่มีน้ำหนักมากที่สุดในการประเมิน probability ของ ROSC และ meaningful survival ระหว่าง prolonged resuscitation
