Cardiac arrest prognosis

Pre-arrest

สามารถประเมิน pre-arrest prognosis ได้จากสภาพของผู้ป่วยก่อนเกิดเหตุ โดยเฉพาะ functional status, อายุ และภาระโรคร่วม (ดัดแปลงแนวคิดมาจาก Go-FARscore) โดยมองผู้ป่วยเป็น 3 มิติ

  1. Functional status ถ้าก่อนเกิด arrest มี Cerebral Performance Category (CPC) 1 หรือ neurological function ปกติ/เกือบปกติ = score – 2
  2. Age ให้คะแนนตามอายุ
    • อายุ > 85 ปี = score +2
    • อายุ 75-84 ปี = score +1
  3. Comorbidities มีแต่ละข้อให้ score +1 ยกเว้น major trauma ให้ score +2
    • Major trauma = score +2
    • Acute stroke
    • Metastatic/hematologic cancer
    • Septicemia
    • Hypotension/hypoperfusion (SBP < 90, MAP < 60 หรือ vasopressor/inotrope หรือ IABP ภายใน 4 ชั่วโมงก่อน arrest)
    • Respiratory insufficiency (P/F <300, PaO₂ <60, SaO₂ <90%, CO₂ >50, RR >40/<5 หรือจำเป็นต้องใช้ NIV/invasive ventilation
    • Renal insufficiency (Cr > 2) / dialysis
    • Hepatic insufficiency (cirrhosis, bilirubin > 2 + AST > 2 x ULN)
    • From skilled nursing facility = Frailty

คิด Cumulative burden ในใจ

  • ถ้าคะแนนรวม > 4  โอกาสรอดจนออกจากโรงพยาบาล พร้อม CPC 1 จะประมาณ 0.9% (ถ้า CPC > 2 ประมาณ < 5%)
  • ถ้าคะแนนรวม 3 โอกาสรอดพร้อม CPC 1 ประมาณ 1.7% (CPC > 2 ประมาณ 7-10%)

การประเมินข้อนี้ มีประโยชน์ที่สุดในการประเมิน pre-arrest probability of meaningful recovery และการพูดคุยเรื่อง goals of care มากกว่าการใช้เป็นเกณฑ์หยุด CPR เพียงลำพัง


Presumptive Cause of Arrest

Etiology มีผลต่อ prognosis เพราะ cardiac arrest บางสาเหตุเกิดจากกระบวนการที่ rapidly reversible ในขณะที่บางสาเหตุเป็นผลสุดท้ายของ irreversible systemic disease

ให้พิจารณา Cause × Reversibility × Time to definitive treatment

  • Coronary cause คือ มี acute coronary occlusion และ primary ventricular arrhythmia เป็นกลุ่มที่โดยทั่วไปมี prognosis ดี โดยเฉพาะถ้าเป็น witnessed arrest + immediate CPR + VF/pVT + early defibrillation (survival 10–30%+) ข้อได้เปรียบ คือ ไม่มีภาวะขาดออกซิเจนมาก่อนหัวใจหยุดเต้น และแก้ไขสาเหตุได้โดยการทำ PCI
  • Drug overdose / poisoning ควร prolonged CPR โดยเฉพาะเมื่อมี antidote, extracorporeal removal หรือ ECMO/ECPR option; ข้อได้เปรียบ คือ ผู้ป่วยมักอายุน้อยกว่า และมี reversible pathology แม้ initial rhythm อาจเป็น PEA/asystole (survival 9–19%)
  • Hypoxia / respiratory arrest พยากรณ์โรคขึ้นกับระยะเวลาของ hypoxia ก่อน circulatory arrest (survival 5-15%) ซึ่งถ้านานจะมี neurological prognosis แย่กว่ามาก เพราะ cerebral hypoxia เริ่มก่อนหัวใจหยุด
  • Pulmonary embolism โดย massive PE มักเกิดเป็น PEA และ overall prognosis ไม่ดี (survival 6–16%) แต่ PE เป็น potentially reversible cause ซึ่งอาจ prolonged CPR หากกำลังรอผลของ definitive therapy (เช่น thrombolysis ระหว่าง CPR, thrombectomy, หรือ ECPR)
  • Drowning พยากรณ์โรค ขึ้นกับหลายปัจจัย (submersion time, water temperature, witnessed status, immediate rescue, early ventilation, degree of hypothermia) ซึ่งมี survival โดยรวม 10–12%
  • Hyperkalemia สามารถทำให้เกิด PEA, asystole หรือ malignant arrhythmia ซึ่งเป็น directly treatable cause ควรทำ aggressive correction
  • Tamponade / tension pneumothorax ทั้งสองเป็น classic reversible causes ซึ่งสามารถเปลี่ยน prognosis ได้ทันทีหากได้รับ definitive treatment
  • Trauma / massive hemorrhage โดยทั่วไป traumatic OHCA มี survival ต่ำ (2–5%) สิ่งสำคัญคือ rapid hemorrhage control + blood replacement + definitive intervention
  • Sepsis arrest โดยเฉพาะในผู้ป่วยที่มี refractory shock + vasopressor + severe acidosis + multiorgan failureเป็นหนึ่งใน phenotypes ที่ prognosis แย่ (survival 7–17%) มักไม่มี single rapidly reversible lesion แต่เป็น culmination ของ progressive systemic failure
  • Intracranial catastrophe (massive ICH/SAH) สามารถทำให้เกิด cardiac arrest และ ROSC ได้ แต่ไม่ได้หมายความว่า neurological prognosis ดี เพราะ underlying catastrophic cerebral injury อาจ irreversible อยู่แล้ว

สรุป

  • Potentially favorable / highly reversible เช่น Primary VF/coronary cause, Drug/toxin, Rapidly reversible hypoxia, Hyperkalemia/metabolic cause, Hypothermia
  • Poor baseline prognosis แต่มี definitive rescue therapy เช่น Massive PE, Tamponade, Tension pneumothorax, Selected hemorrhage/trauma
  • Generally unfavorable เช่น Refractory septic shock/MOF, Uncontrolled massive hemorrhage, Prolonged asphyxia/hanging, Catastrophic intracranial hemorrhage, Irreversible terminal disease


Intra-arrest Findings

เมื่อ CPR ดำเนินไป prognosis ไม่ได้คงที่ แต่สามารถ update ได้จากข้อมูลที่เกิดขึ้นระหว่าง resuscitation

ข้อมูลเหล่านี้ประกอบด้วย

rhythm evolution, ETCO₂, POCUS cardiac activity, blood gas/POCT, lactate trend และ response ต่อ resuscitation

หลักสำคัญคือ

ไม่มี intra-arrest finding ใดเพียงตัวเดียวที่มีความแม่นยำเพียงพอสำหรับการตัดสินใจหยุด CPR

แต่เมื่อ independent markers หลายตัวชี้ไปในทิศทางเดียวกัน ความมั่นใจในการประเมิน prognosis จะเพิ่มขึ้น

 

1. Initial rhythm และ rhythm evolution

โดยทั่วไป prognosis เรียงจาก VF/pVT > PEA > asystole แต่ rhythm ไม่ควรถูกมองแบบ static เพราะการเกิด shockable rhythm หรือ transient circulation ระหว่าง resuscitation แสดงว่าหัวใจยังมี physiological responsiveness

แต่ในทางตรงกันข้าม ถ้า persistent asystole ตลอด prolonged high-quality CPR เป็น adverse prognostic sign ที่สำคัญ

 

2. ETCO₂

เนื่องจากเมื่อ ventilation คงที่ ปริมาณ CO₂ ที่มาถึงปอดขึ้นกับ pulmonary blood flow จึงสามารถใช้เป็น indirect marker ของ cardiac output จาก chest compression ได้

Classic studies พบว่าผู้ที่เกิด ROSC มี ETCO₂ สูงกว่าผู้ที่ไม่ ROSC อย่างมาก

ในรายที่มี Persistent ETCO₂ <10 mmHg หลัง CPR ประมาณ 20 นาที สัมพันธ์กับ extremely poor prognosis

แต่ subsequent studies พบว่า cutoff นี้ไม่ได้มี specificity 100%

ดังนั้น ETCO₂ <10 ไม่ควรถูกใช้เป็นเหตุผลเดียวในการ terminate resuscitation

ต้องตรวจสอบด้วยว่า

  • airway/ET tube ถูกต้อง
  • ventilation ไม่มากเกินไป
  • chest compression มีคุณภาพ
  • ไม่มี massive PE หรือ pulmonary blood-flow obstruction
  • ไม่มี technical problem

 

3. POCUS cardiac activity

POCUS สามารถแยก PEA ออกเป็นสอง physiological phenotypes

Pseudo-PEA: มี organized cardiac contraction แต่คลำ pulse ไม่ได้

กับ

True PEA/cardiac standstill: ไม่มี meaningful myocardial movement

การเห็น cardiac activity ระหว่าง pulse check สัมพันธ์กับ ROSC และ survival ที่สูงกว่า cardiac standstill อย่างชัดเจน

ดังนั้น

PEA + cardiac motion

มี prognosis ดีกว่า

PEA + complete cardiac standstill

แต่ cardiac standstill ไม่ได้มี specificity 100% สำหรับ death จึงไม่ควรใช้ ultrasound เพียงอย่างเดียวเพื่อหยุด CPR

POCUS มีประโยชน์เพิ่มเติมในการค้นหา reversible cause เช่น

tamponade, RV strain/possible PE, severe hypovolemia และ pneumothorax

โดยต้องทำโดยไม่ทำให้ chest compression interruption ยาวขึ้น

 

4. pH / Blood gas (adjusted OR for no-ROSC 0.03 ต่อการเพิ่ม pH 1 unit)

Severe acidosis ระหว่าง CPR สัมพันธ์กับ probability of ROSC และ survival ที่ลดลง

pH สะท้อนทั้ง

  • duration of arrest
  • adequacy of perfusion
  • ventilation
  • underlying metabolic state

ดังนั้น progressively falling pH despite adequate CPR เป็น adverse sign

แต่ในบางกลุ่มอาจเป็นส่วนหนึ่งของ reversible pathology (poisoning, hyperkalemia, hypothermia) ซึ่งยัง recover ได้

 

5. Potassium (OR 2.28)

Hyperkalemia อาจเป็น สาเหตุของ arrest หรือ เกิดจาก prolonged arrest ทำให้เกิด prolonged ischemia (cellular injury, severe acidosis) ต้องตีความตามบริบท เช่น

  • ESRD + missed dialysis + K 8.5 + PEA เป็น potentially reversible arrest
  • Prolonged unwitnessed arrest + K 8.5 หลัง CPR 45 นาที อาจสะท้อน severe systemic ischemia

 

6. Lactate

Initial lactate สูงสัมพันธ์กับ poor outcome ในหลาย studies แต่ discrimination ของ single value ไม่ดีพอสำหรับใช้เดี่ยว ๆ แต่การที่ lactate ลดลงระหว่าง ongoing CPR อาจสะท้อน effective circulation และ metabolic recovery ดังนั้น

Trend จึงมีประโยชน์มากกว่า

  • Lactate stable/decreasing + pH improving = Favorable pattern
  • Lactate progressively rising + pH progressively falling = Unfavorable pattern

อาจเจาะ POCT ซ้ำทุก 10 นาที: ทุก lactate ที่ลดลง 1 mmol/L สัมพันธ์กับ sustained ROSC เพิ่มขึ้นประมาณ 16% (aOR 1.16))

อย่างไรก็ตาม lactate ยังขึ้นกับ pre-arrest shock, sepsis, seizures, epinephrine และ hepatic clearance จึงต้องตีความร่วมกับบริบท


Dynamic Cardiac Arrest Prognosis Profile

การประเมินระหว่าง CPR จึงไม่ควรถามว่า

“มี marker ตัวไหนที่บอกให้หยุด?”

แต่ควรถามว่า

“ข้อมูลทั้งหมดกำลัง converge ไปในทิศทางใด?”

Favorable intra-arrest profile

VF/pVT หรือ rhythm improving

  • ETCO₂ >20–25 mmHg หรือเพิ่มขึ้น
  • cardiac motion on POCUS
  • pH stable/improving
  • lactate stable/decreasing
  • transient ROSC/physiologic response

→ ongoing resuscitation ยังมี physiological rationale สูง

Highly unfavorable intra-arrest profile

Persistent asystole

  • no cardiac motion repeatedly
  • ETCO₂ persistently <10 despite verified high-quality CPR
  • progressively severe acidosis
  • rising lactate
  • no transient ROSC or rhythm improvement

→ probability of successful resuscitation ลดลงอย่างมาก โดยเฉพาะเมื่อไม่มี reversible cause

แต่แม้ profile นี้ก็ยังต้องตีความร่วมกับ cause เช่น hypothermia, poisoning, PE หรือ correctable metabolic disturbance


Integrating ทั้ง 3 Prognosis Profiles

การประเมิน cardiac arrest ที่เหมาะสมจึงควรประกอบด้วยสามชั้น

1. Baseline prognosis

Functional status + age + frailty + comorbidities

↓

2. Arrest prognosis

Cause + reversibility + witnessed status + no-flow time + initial rhythm

↓

3. Dynamic intra-arrest prognosis

CPR duration + rhythm evolution + ETCO₂ + POCUS + pH/K/lactate + response to treatment
